The Metabolic Optimization Continuity Program is a personalized medical and lifestyle support program designed to help individuals improve their metabolic health, achieve sustainable weight loss goals, and build long-term healthy habits. Treatment plans are individualized and may include nutrition guidance, lifestyle modifications, lab review, behavioral strategies, and medication management when clinically appropriate.
Following your Comprehensive Metabolic Wellness Evaluation, we will work together to determine whether ongoing continuity support is appropriate based on your goals, medical needs, and stage of your health journey.The Metabolic Optimization Continuity Program is designed to provide that continued support through a personalized, whole-person approach to care.
Program Cost: $175/month

The Metabolic Optimization Continuity Program is intended to provide ongoing support and continuity of care. Participation does not guarantee prescriptions, laboratory testing, specific treatments, or particular outcomes. All recommendations are based on clinical judgment, medical appropriateness, and individual patient needs.
Medication costs, laboratory testing, pharmacy services, and diagnostic testing are not included in the monthly program fee and are billed separately by the respective providers.

The Metabolic Optimization Continuty Program is offered on a month-to-month basis and designed to provide ongoing support based on your individual goals and clinical needs.
To continue participating in your program, the monthly program fee is due prior to the start of the next support month. Once payment is received, eligible follow-up appointments and ongoing program support can be scheduled for that period.
There is no long-term contract or required commitment. Participation is entirely voluntary, and patients may choose whether to continue from month to month based on their treatment goals, progress, and preferences.
Medication Refills and Ongoing Monitoring: To ensure safe and appropriate prescribing practices, patients receiving prescription medications through the Metabolic Wellness Continuity Program are generally expected to remain actively engaged in the program, attend recommended follow-up appointments, and complete any requested monitoring (such as laboratory testing, vital sign assessments, or other clinically indicated evaluations).
Failure to participate in recommended follow-up care or required monitoring may result in delays, modifications, or discontinuation of medication prescribing until appropriate evaluation can occur. Prescription renewals are provided at the discretion of the treating clinician based on clinical appropriateness and safety considerations and are not guaranteed.

Not all patients are candidates for medication, and medication recommendations are based on medical history, clinical assessment, and shared decision-making.
